FFCRA COVID-19 Related Leave Form

If you have a COVID-19 related leave, please fill out this form and email to Vivian Dassay at vdassay@clare.k12.mi.us.

FFCRA expired December 31, 2020; the district's voluntary extension ended March 31, 2021.  However, staff must still complete the form for tracking purposes.
